Muscles grow only during recovery, not while you lift. The workout's job is to deliver a stimulus; the body's job in the following days is to rebuild stressed fibers so they can handle the same challenge more easily, and that rebuild is where new muscle appears.
When a muscle is placed under enough tension, the strain is detected by mechanosensors in the cell wall. That triggers a cascade: calcium is released, a protein called mTOR is activated, and muscle stem cells (satellite cells) fuse to the damaged fiber, donating their nuclei. With more nuclei, the fiber can produce more contractile protein, which is laid down as new myofibrils packed in parallel. This is why a small increase in tension matters: the body only adapts when the strain exceeds what it has already adapted to.
Suppose you squat 100 kg for 5 repetitions and 3 sets. Total work is 1500 kg. Next week you add 2.5 kg to the bar. The same 5x3 now totals 1537.5 kg, an increase of just 2.5%. More importantly, each individual muscle fiber experiences slightly more tension at the start of each rep. That tiny bump is enough to trigger the mechanosensitive pathways. You could also add one rep to one set, or simply rest less between sets, as long as the number of hard reps goes up. The specific variable matters less than the fact that the muscle is asked to do something it has not done before.
Delayed onset muscle soreness, the stiffness you feel a day or two after a new session, is mostly inflammation from unusual eccentric contractions and micro-tears. It can be severe after a brand-new movement but lead to little growth. Conversely, a familiar lift at a slightly heavier weight may cause almost no soreness while still producing substantial adaptation. Tracking the weight on the bar and the number of reps you complete is a far more reliable sign of progress than how tender your legs feel.
In the first weeks of training, strength gains come largely from the nervous system learning to recruit muscles, not from new tissue; you can get stronger without much visible growth. In advanced lifters, the opposite problem appears: even with solid training, growth slows because the muscle has already adapted to near its limit. And none of this works without raw materials: if protein intake is too low, or sleep is cut short, the rebuilding process is impaired, and overloaded muscle may simply remain broken down rather than growing. The formula of tension, protein, and rest is correct, but each part has to be adequate for the person's current level.
Cram I work out hard every day but my muscles barely grow. Why?
Rep Because muscles don't grow during the workout. That is the most common myth.

Rep During recovery. Lifting tears muscle fibers. Your body rebuilds them stronger while you rest.
Cram So I should take more rest days?

Rep Either one. A small increase in load or volume signals your body to add more protein strands.
Cram So soreness does not mean growth?

Cram And I need protein for the repair?
Rep Yes. Without enough protein your body cannot build new tissue. Train, eat, sleep, repeat.
